To apply for recertification, complete the following steps: 

1. Enter your training hour details using the embedded form below, and click the Submit button. Alternatively, they can be uploaded in the section following the form.
2. Submit your registration payment by clicking the add to cart button at the bottom of this page.

Please note that applications from Canadian certification holders should be submitted through our partners at Inform Canada.
