As communities face increasingly complex social and health challenges, traditional referral models are evolving. In this session, Pam Hillier will share Community Connections journey from information and referral to a systems navigation approach grounded in partnership, shared responsibility, and coordinated care.


Drawing on research, pilot projects, and partnership initiatives, Pam will highlight Community Connections experiences with social prescribing, eReferral systems, and closed-loop reporting, and how these efforts have strengthened connections between primary care, community services, paramedic services, and police partners.

Pam Hillier is a community sector leader specializing in systems navigation, partnership development, and shared services. She is passionate about helping organizations work together to create more connected, person-centred systems of support. Through her leadership at Community Connection, Pam has advanced collaborative approaches that improve access to services, strengthen community capacity, and build sustainable solutions to complex social challenges.
