Mapping Caregiver Navigation Services

Join USAging to learn all about caregiver navigation and how to help caregivers connect to the right services at the right time. This webinar will highlight USAging’s work to understand and develop best practices for caregiver navigation as part of the National Strategy to Support Family Caregivers. This session will feature key insights and promising opportunities from USAging’s environmental scan to enhance caregiver navigation across the Aging Network. 

At the conclusion of this session, participants will be able to:

  • Understand the concept and service components of caregiver navigation provided by the Aging Network through Older Americans Act-funded Family Caregiver Support Programs.

  • Identify strengths and opportunities to enhance caregiver navigation services in their organization.

  • Integrate replicable caregiver navigation strategies identified by the environmental scan

Presenters:

• Darya Rahbar, Program Manager , USAging

Darya Rahbar serves as Program Manager at USAging for Caregiver Navigation Services, advancing research, resource development, and evaluation to strengthen caregiver navigation across Older Americans Act Title III-E and VI-C programs that support family caregivers nationwide.

Before joining this project, Darya supported the Dementia Friendly America and Community Care Corp projects at USAging.

• Zachary Trammel, Program Manager , USAging

Zach Trammel currently serves as Program Manager at USAging for Caregiver Navigation Services, advancing research, resource development, and evaluation to strengthen caregiver navigation across Older Americans Act Title III-E and VI-C programs that support family caregivers nationwide.

Before his current work, Zach supervised the national Eldercare Locator, which connects older adults and caregivers to local resources.  He is an InformUSA Certified Community Resource Specialist in Aging and Disability.
