• The Community Connects Desk (CCD), located within the City of Rochester Courthouse, offers in-person service navigation to individuals during court hours. Community members may be referred by judges or other court personnel to meet with a Community Navigator, who provides real-time information and referral services based on their immediate needs. The Navigator also educates individuals on the local Community Information Exchange, helping them gain greater control over their personal information and empowering them to self-navigate available resources. The CCD has supported community members in accessing housing, food, and legal services—making help available at the moment it’s needed most. To date, the program has served over 500 individuals and continues to be a valuable asset to the community.
